Our new School Bridge Social Worker is responsible for the facilitation of social work and case management services to participants of Hudson Guild’s K-8 after school program. Including multiple program sites across Chelsea, the Upper West Side, and their families.

In order to be considered, you need the following:

  • Master’s Degree in Social Work
  • Seminar in Field Instruction (SIFI) certification required
  • Four years of significant experience working with at-risk youth.

In order to impress us, you will also have the following:

  • Strong organizational and communication skills
  • Proficiency in computers.
  • Bilingual in Spanish/English.
  • Experience serving children and their families in a school and community center setting, as well as knowledge of community and agency needs.

RESPONSIBILITIES:

Individual Work:

  • Establish positive relationships with participants, school staff, program staff, and families.
  • Meet on an individual basis with youth as needed.
  • Work collaboratively with all School Bridge staff to identify children in the after-school program and/or their families who need support and case management services.
  • Attend monthly and quarterly conferences with school staff to identify children in the after-school program and/or their families who need support and case management services.
  • Monitor MSW interns facilitate specialized groups for participants such as bullying, conflict resolution, anger management, and facilitate groups when necessary.
  • Work closely with School Bridge Coordinator daily to oversee program needs.
  • Create, implement and track behavior improvement plans along with MSW interns, staff, and families.
  • Keep Social Work Coordinator up-to-date (verbally and in writing) of client and/or program related issues, problems, concerns.
  • Provide crisis intervention.

Family Assistance:

  • Create and maintain engagement with families to provide psycho-educational workshops/groups along with MSW interns monthly.
  • Collaborate with School Bridge Coordinators to create annual Family Calendar.
  • Maintain regular email blast communication with families regarding family events etc.
  • Assist families finding appropriate Mental Health services, access entitlements, benefits, resources, and concrete services for families as deemed appropriate.
  • Establish, foster and maintain cooperative relationships with referral agencies, schools, medical facilities/individuals, organizations and resources as deemed appropriate.
  • Maintain complete, current and accurate records of participants and families receiving case management services.

Staff Training:

  • Assess after school staff needs and seek resources to provide appropriate training.
  • Role model appropriate techniques for effectively working with at risk youth.
  • Provide training and technical assistance to Guild programs and staff on best practices for working with challenging youth.
  •  Track incidents and follow-up.
  • Supervise MSW intern’s individual caseloads, agency process notes, and Graduate School process recordings.
  • Maintain knowledge base of social-emotional learning standards and current practices.

Other Responsibilities:

  • Ensure the proper administration and collection of Social Emotional assessments.
  • Examine data from assessments and inform program modifications.
  • Act as a liaison with mental health clinic. Attend all case conferences.
  • Generate monthly statistics and reports in a timely and accurate manner.
  •  Ensure program compliance with Hudson Guild policies and procedures.
  • Participate in staff development workshops, staff meetings and supervision meetings.
  • Participate in community-building activities sponsored by program and agency and encourage participants to support these activities. 
  • Participate in agency-wide initiatives and committees
  • Perform other program-related duties as assigned.
